The Dlaserca solution empowers people with heart failure to take control and manage their condition effectively. They can track symptoms, set reminders to stay on top of medications, and discover healthy lifestyle tips to improve their health and well-being. Patients can connect within the moderated peer-to-peer community to exchange information, share tips, and offer advice, and are motivated to share data with their care team.
The outcome
The result is a user-friendly solution designed to improve health literacy, facilitate collaboration with the care team through data sharing and providing essential community support to patients. The solution is currently deployed to patients and will hereafter be evaluated to assess the effectiveness of improving quality of life and achieving positive healthcare outcomes, while reducing hospital visits.
